The Student Health Care Pre-Information Form is intended for students enrolled in upper secondary vocational education or high school. The form is used to gather preliminary information for the health nurse's health check. Please complete the form prior to the health check.
Please note that if you have filled out and submitted a paper form, you do not need to fill out the electronic form again. For additional information about the health check and the form, please contact your school's health nurse. It is important to note that forms will not be processed during the period from June 21 to July 28, 2024.
